dc.description.abstractMonitores oscilométricos para mensuração da pressão arterial desenvolvidos exclusivamente para a medicina veterinária têm sido utilizados com frequência na rotina clínica e anestésica, fazendo-se necessário validar a performance em relação ao método invasivo nas diferentes condições hemodinâmicas. O presente estudo objetivou avaliar a concordância entre o monitor PetMap e o método invasivo na mensuração da pressão arterial em 3 regiões distintas (acima do carpo, acima do tarso e base da cauda), em cães anestesiados, sedados e conscientes, considerando os critérios estabelecidos pelo ACVIM. Na primeira etapa do estudo, valores de pressão arterial sistólica (PAS), média (PAM) e diastólica (PAD) foram obtidos simultaneamente de forma invasiva e não invasiva (com o PetMap) em 8 cães anestesiados e, subsequentemente, sedados. Em ambas as situações, diferentes condições hemodinâmicas (hipotensão, normotensão e hipertensão) foram induzidas de forma aleatória. Na segunda etapa, as mensurações foram realizadas em cães conscientes (sem a influência de fármacos sedativos ou anestésicos) na condição hemodinâmica vigente. Os resultados foram analisados de acordo o método modificado de Bland-Altman. De maneira geral, os valores de PAM e PAD obtidos com o PetMAp apresentaram melhor concordância com o método invasivo do que os valores de PAS. Nos cães anestesiados, o PetMap apresentou boa concordância com o método invasivo nas condições de normotensão e hipotensão (condições mais frequentes durante o período anestésico) apenas com o manguito posicionado na região da base da cauda. Nos cães sedados e conscientes, o desempenho do PetMap foi adequado e superior do que durante a anestesia, nas regiões acima do carpo e base da cauda em decúbito lateral, podendo ser adotada a última região, nos animais conscientes na posição quadrupedal. A região acima do tarso, não é recomendada para a mensuração da pressão arterial com o PetMap em nenhuma dessas situações, devido a baixa concordância ao método direto.pt_BR

dc.description.resumoOscillometric monitors developed exclusively for veterinary medicine have been frequently used in anesthetic and clinic routine, making it necessary to validate the performance compared to the gold standard in different clinical conditions. The present study aimed to validate the PetMap monitor in 3 distinct regions (above the carpus, above the tarsus and the base of the tail), in anesthetized, tranquilized and conscious dogs, considering the criteria established by the ACVIM. In the first stage of the study, systolic (SAP), mean (MAP) and diastolic (DAP) blood pressures were obtained simultaneously form invasive and non-invasive method (with the PetMaP) in 8 anesthetized dogs, and, subsequently, sedated. In both situations, different hemodynamic conditions (hypotension, normotensive and hypertension) were induced at random and thus blood pressure values were obtained. In the second step, the measurements were carried out in conscious dogs (without the influence of sedatives or anesthetic drugs) without changing the existing hemodynamic condition though. The results were analyzed according to the modified method of Bland-Altman [bias = indirect method (PetMap) – gold standard (invasive method)]. In general, the obtained MAP values with PeMap showed better agreement with invasive method than the values for SAP and DAP. In anesthetized dogs, the PetMap showed good agreement with the invasive blood pressure during conditions of hypotension and normotensive (most frequent conditions during anesthesia period) only with the cuff positioned in the base of the tail. In sedated and conscious dogs, PetMap performance was adequate and superior compared to the anesthesia period, in the regions above the carpus and base of the tail in lateral recumbency, and may be adopted the last region in conscious standing dogs. The region above the tarsus is not recommended for blood pressure measurement with the PetMap in any of those situations due to high values of bias and standard deviations obtained for all blood pressures (SAP, MAP, DAP).pt_BR
